Download and save a copy of the form and complete it on- or offline. To submit a form online, connect to the internet and click the Submit button on the last page. Click Submit once. You will receive a confirmation number within a few minutes.

Authorize your customer contacts

To confirm signing authorities for your business and to authorize third-party contacts to discuss and exchange information with Agricorp on your behalf, submit an Authorized customer contacts form to Agricorp. Please be prepared to enter your Agricorp ID. AgriStability participants will find their Agricorp ID printed on their personalized AgriStability application, or will receive their Agricorp ID in the mail in January. If you do not participate in AgriStability and need to update your third-party authorizations, please call Agricorp at 1-888-247-4999.

Application for new customers

For partnerships, submit only one form per partnership. If submitting forms online, you don't have to sign.

You must have a valid premises identification number. Your application cannot be processed until this number is provided to us. To obtain your premises ID, apply online at www.ontarioppr.ca or call 1-855-697-7743. Have your assessment roll number handy. Your assessment roll number can be found on your property tax assessment.

Managing your coverage

  •  Customer change notice (to report changes to your name, address, business structure, partners or shareholders)
  •  Policy change request (to notify Agricorp about a change in your projected sales or to cancel enrolment in a category)

Renewal for current customers

Balances owed to the Crown

Agricorp is required to recover balances owed to the Crown, including any RMP overpayments that may occur. Overpayments may occur as a result of incomplete program applications, processing errors, changes to farm operations and the nature of advance payments to producers in financial distress.

If you have a balance owing, you must repay the balance, including interest, within three years of the date you were notified about the debt. You must also submit a repayment plan outlining how you plan to repay the debt. Interest will be charged on any outstanding balance owing for more than 30 days.

Agricorp also recovers overpayments from any program payments you may be eligible to receive. As such, RMP overpayments may be recovered from other program payments and overpayments from other programs may be recovered from your RMP payment.
