Download and save a copy of the form and complete it on- or offline. To submit a form online, connect to the internet and click the Submit button on the last page. Click Submit once. You will receive a confirmation number within a few minutes.

Sales report

You have a choice of two methods for submitting your sales report:

Download a personalized, fillable sales report that contains your customer information. You can complete this form offline and then submit it electronically with the click of a button. Please be prepared to enter your Agricorp ID and Livestock policy number.
Download a blank sales report.

Application for new customers

For partnerships, submit only one form per partnership. If submitting forms online, you don't have to sign.

2012 Application - RMP for livestock

You must have a valid premises identification number. Your application cannot be processed until this number is provided to us. To obtain your premises ID, apply online at www.ontarioppr.ca or call 1-855-697-7743. Have your assessment roll number handy. Your assessment roll number can be found on your property tax assessment.

To complete the following forms, please download the PDF file and print it. The Customer change notice is used to report changes to your name, address, business structure, partners or shareholders.

The renewal deadline was March 16, 2012.
